You can download the application, complete it and submit by email or mail to Focus on Energy by the information listed on the bottom of the application. Please keep one copy for your records.

  If you are filling out a Steam Trap Incentive Application, please also fill out and submit this Steam Trap Survey and Repair Log.
• Boiler Combustion Management System Incentive Application
• Commercial Ground Source (Geothermal) Heat Pump

Equipment Qualification
If a model you plan to sell is not on the pre-qualified list, you should complete an Equipment Qualification Form and fax to: Business Program - HVAC (fax: 608.249.0307) to have the model pre-qualified. This will avoid promising a customer a reward for equipment which may not be eligible to receive a reward.
