Spreadsheet Requirements

Applying for a business incentive that requires more information? Please fill out the following spreadsheet based on the incentive you are applying for and submit with your incentive application.

1. All site locations must be listed under the same FEIN (Federal Employer Identification Number).
2. All equipment/services listed must be the same type (all controls, all CFL's, etc.)
3. All required information must be completed on the spreadsheet.
