There are two options for sending in your Reward form to Focus on Energy.
- You can download and print out an application and then mail it to Focus on Energy.

If equipment was installed in a HOME click here: download Residential application
If equipment was installed in a BUSINESS click here: download Business application
- You can fill out the application online.
NOTE: You will still need to mail in a copy of the paid invoice within 30 days of installation or fax the paid invoice
to 608.249.0339
Attn: HVAC Rewards. A copy of the invoice must be received before the reward application can be processed.
