There are two options for sending in your Reward form to Focus on Energy.*

1. You can download and PRINT out an application and then MAIL
   (See address on form)
             If equipment was installed between January 1 and June 30, 2010 in a HOME click here
             If equipment was installed between July 1 and December 31, 2010 in a HOME click here
             If equipment was installed in a BUSINESS click here
   
   
2. You can fill out the Residential application ONLINE.
   NOTE: A copy of the paid invoice must be mailed or faxed to Focus on Energy within 30 days of installation.
   The reward application can not be processed until the invoice is received.
