If you are not currently participating in the Program, please complete and sign
a Residential Market Provider Agreement and fax it to
608.249.0339 ATTN: Efficient Heating & Cooling Program.
If you submitted a Market Provider Agreement for previous years of the Efficient Heating & Cooling Program, we will
extend that agreement rather than require you to submit a new form.
Please submit a new form if you have changes to your business (i.e. changes in address, phone number or contact information).

at 608.249.0339. Please allow up to 10 business days for materials to arrive. Consumer Material Request Form
Equipment Qualification
If a model you plan to sell is not on the pre-qualified list, you should complete an Equipment Qualification Form and fax to:
Efficient Heating & Cooling Program (fax: 608.249.0339) to have the model pre-qualified. This will avoid promising a customer
