Market Provider Agreement
This agreement is required before a contractor may offer rewards to consumers. To participate in Program Year 2007-2008, commencing January 1, 2007:
  • If you submitted a Market Provider Agreement for previous years of the Efficient Heating and Cooling Program, we will extend that agreement rather than require you to submit a new form. By accessing Cash-Back Rewards in 2007-2008, you will have agreed to abide by the terms of the Market Provider Agreement and the program requirements.
  • Please submit a new form if you have changes to your business (i.e. changes in address, phone number or contact information).
  • If you are not currently participating in the Program, after reading the Program Guide, please complete (including signing) a Residential Market Provider Agreement or BP Ally Application (located in the program binder and online) and fax it to 608.249.0339 ATTN: Efficient Heating and Cooling Program. Only a participating contractor can offer the Program's Cash-Back Rewards to customers.

Efficient Heating and Cooling Program Rewards
A reward form must be submitted with the invoice for the customer to receive a reward. This form is used for any qualifying residential installation.

The form below may be filled out and attached to the Reward Application instead of filling out the Charge and Airflow data in the Split System Information Section of the Reward Application. If you would like original copies of these forms, which are printed on cardstock, please fax a Material Request to: Efficient Heating and Cooling Program (fax 608.249.0339).

Equipment Qualification
If a model you plan to sell is not on the pre-qualified list, you should complete an Equipment Qualification Form and fax to: Efficient Heating and Cooling Program (fax: 608.249.0339) to have the model pre-qualified. This will avoid promising a customer a reward for equipment which may not be eligible to receive a reward.
