New to the Program
If you are not currently participating in the Program, after reading the Program Guide, please complete (including signing) a Residential Market Provider Agreement or BP Ally Application (located in the program binder and below) and fax it to: 608.249.0339 ATTN: Efficient Heating and Cooling Program. Only a participating contractor can offer the Initiative's Cash-Back Rewards to customers.

Continuing Contractor
If you have already submitted a Contractor Participation Agreement or BP Ally Application for the Program, we will simply extend that agreement rather than require you to submit a new form. By accessing Cash-Back Rewards and incentives, you will have agreed to abide by the terms of the Program requirements.

Please submit a new form if you have changes to your business (i.e. changes in address, phone number or contact information).
